In order for your child to be eligible to participate in our league, he/she must live or attend school within our boundaries.  Therefore, during the registration process you will be asked to provide proof of your child's home address or school enrollment.  Listed below are two ways to provide this documentation.

School Enrollment Form- Completed by parent/guardian AND school principal or administrator.  This form will be used every year your child registers with Elyria Little League unless they change schools, at which time a new form will be required.  To access this form, please click the link below, print the form, and ask your child's principal or administrator complete.

Groups 1, 2, and 3 Proofs of Residency- One document is required from each group, examples can be found by clicking the link below.  Documents MUST be dated between February 1, 2018 and January 31, 2019.  These documents are required each year your child registers.
